Geriatric Case Management

Geriatric Case Management (GCM) is a collaborative process that helps senior citizens, their families and their caregivers effectively address changes and needs that can interfere with healthy aging. GCM begins with a comprehensive assessment of the senior's medical, psychosocial and financial needs, and includes attention to the roles and responsibilities family members want to have in helping their loved one maintain as healthy and satisfying life as possible.

Not all situations seniors and/or their families encounter have to have an associated medical condition or diagnosis in order to benefit from the services of a specialist like Allie Hafez. Maintaining a healthy and active lifestyle can prevent or delay the development of medical problems; and as a GCM who is also an Occupational Therapist, Allie has the education and skills to identify and address the often-subtle components of function throughout the lifespan.

The unusual combination of being both a licensed Occupational Therapist (OTR/L) and a Certified Case Manager (CCM) gives Allie the ability to help seniors, their families and/or caregivers maximize the quality of their lives at any age.
