What is Case Management?
Case management is a specialty practiced by several types of health and human service professionals. These professionals, who are often licensed and/or certified as occupational therapists, social workers, rehabilitation counselors, psychologists or nurses, provide services designed to help people reach the highest possible level of wellness and functional ability.
The Commission for Certification of Case Managers (CCMC) defines Case Management as "a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client's health and human service needs. It is characterized by advocacy, communication, and resource management and promotes quality and cost-effective interventions and outcomes. CCMC also describes the philosophy of case management as "ensuring that the [service] provided is safe, effective, client-centered, timely, efficient, and equitable"
Case managers help individuals and service providers to identify the resources that provide the most cost-effective outcomes, based on the individuals' needs and values.
